Registration for Selected Players U13-U18

If your child has been selected, please click on Player Registration and register for the season. See below for specific instructions.

Parents of all registering players are requested to print and sign all of the forms on our registration page and bring them to Central Park concession stand on Wednesday, June 15th, or Thursday June 16th, from 6:30PM til 8:30PM to complete your registration. Please bring your proof of credit card payment or a check. For a $25 fee, payment may be made in two installments. If you are paying by credit card, you also have the option to pay in two installments.

You may pay by credit card or check. At least 50% of the annual fee should be paid by July 15th, and the balance by September 30th. If you need to make special financial arrangements, please contact our treasurer, Lee Levenberg, at neonleon2000@email.com, or call between 5:30PM and 9PM, 954-558-3080.

If you are out of town this week, we plan to schedule another registration session towards the end of June. If you have any questions about tryouts for U13-U18, or about our academy program for U9-U12, please contact John Ramos at 954-448-3571.

Registration for Players U9-U12

All players ages U9 through U12, whether or not they attended tryouts, are also invited to register for the upcoming season. Players age is determined on July 31st. If a child is 8, 9, 10 or 11 years old on July 31st they are U9 to U12. If your child is 7 years old on July 31st and is going into 3rd grade or if you feel your 7 year old is otherwise ready for club soccer, please contact our Director of Coaching, John Ramos at 954-448-3571.

2011-2012 Travel Tryout and Registration Info

Player Registration: (selected players)

Any player that does not complete the registration process will not be guaranteed a spot on a team. Open spots may be offered to players on the waiting list. Players that cannot attend must make other arrangements directly with their coach. Players must attend registration to be sized for their uniform.

All forms must be completed and submitted at registration. Players must complete the online player/team registration process prior to attending registration. Payment or a financial assistance forms must also be submitted at registration. All forms and player registration requirements are available on the website.

STEP 1: City of Plantation Travel Card

All selected players are required to obtain a travel card from the City of Plantation.
-Plantation Residents - $30 (picture ID required, also present a utility bill to verify residence)
-Non-Residents - $80 (picture ID required)
*City card is good for 2011-2012 PFC Season (1 year)

Registration will be available through this website in the online registration section.

STEP 2: Registration

Required Documents at Registration: (Player must register online for their age group after they are selected)

1. Check or receipt to confirm online payment. (Financial Assistance Application, if applicable)

2. 2 passport sized photographs.

3. Copy of birth certificate or passport (players new to FYSA)

4. FYSA Player Registration Form

5. Notarized Medical Release

6. Signed FYSA Code of Ethics

7. Signed Player/Parent Agreement

8. Signed Financial Expectation Agreement

9. PAL Registration Form

10. All players 12 years old and up with foreign birth certificates will require international clearance or waiver. If you are already with PESC and did this previously, there is no need to do it again. The forms are available on the Eagles web site under forms.
