Registration for Selected Players 2012-2013 Season - new process, please read!!

If your child has been selected, please click on Player Registration and register for the season. IT IS VERY IMPORTANT TO ENTER OR UPDATE A VALID EMAIL ADDRESS - ALL FORMS WILL BE SENT FOR REGISTRATION, ELECTRONICALLY.  See below for specific instructions.

Online Player Registration 

You may pay by credit card or check.  At least 50% of the annual fee should be paid by July 15th, and the balance by September 30th.  If you need to make special financial arrangements, please contact our treasurer, Lee Levenberg, at neonleon2000@email.com, or call between 5:30PM and 9PM, 954-558-3080.

**ONCE YOU HAVE REGISTERED ONLINE, YOU WILL RECEIVE THE FORMS FOR REGISTRATION VIA "DOCUSIGN" - YOU MUST ELECTRONICALLY SIGN ALL OF THE FORMS RECEIVED. THE MEDICAL RELEASE WILL NEED TO BE PRINTED, NOTARIZED AND GIVEN TO YOUR TEAM MANAGER AT THE BEGINNING OF YOUR SEASON.  WE WILL NOT BE COLLECTING THEM.  IF YOU HAVE ANY PROBLEMS WITH THE ELECTRONIC DOCUMENTS, PLEASE CONTACT MICHELLE MALONE AT MICHELLE.MALONE@COMCAST.NET

PLEASE NOTE - YOU MUST REGISTER ONLINE BEFORE YOU WILL RECEIVE YOUR REGISTRATION DOCUMENTS!!!

We will be announcing dates, where we will collect check payments at the park, if you are not able to make an online payment.  Your player registration will not be complete until the following steps are taken:

1.  Register online (ensure email address is correct and valid)

2.  Make your payment online or in person (checks only) - esnure City of Plantation travel card is purchased online as well - do not purchase in person, must be online.

3.  Receive your registration documents online from Docusign

4.  Complete registration documents and print medical release

SPECIAL INSTRUCTIONS FOR FILLING OUT THE FORMS:

A.  ANYWHERE IT SAYS "PLAYER SIGNATURE" ON THE FORM, SIMPLY TYPE IN YOUR PLAYER’S NAME. 

B.  ALL BOXES MUST HAVE A RESPONSE, BEFORE IT WILL ALLOW YOU TO FINALIZE THE FORMS.  IF THERE IS SOMETHING NOT APPLICABLE OR THE ANSWER IS UNKNOWN, PLEASE TYPE IN N/A.

5.  If you are new to club soccer, you must email a birth certificate to Michelle.Malone@comcast.net.  If the player is foreign born and has not been cleared to play in the US via any other club, there are supplemental forms to fill out.  Please contact Michelle with questions and clarification.

Registration for Players U9-U12

All players ages U9 through U12, whether or not they attended tryouts, are also invited to register for the upcoming season.  Players age is determined on July 31st.  If a child is 8, 9, 10 or 11 years old on July 31st they are U9 to U12.  If your child is 7 years old on July 31st and is going into 3rd grade or if you feel your 7 year old is otherwise ready for club soccer, please contact our Director of Coaching, John Ramos at 954-448-3571.
